Where Does This Pain Come From?
Hip and buttock pain can make everyday activities surprisingly difficult. Walking, climbing stairs, sitting through a meeting, getting out of the car, or even sleeping on one side can become painful. While many people assume the hip joint is to blame, the true source of pain isn't always where you feel it.
The hip is surrounded by powerful muscles, tendons, ligaments, bursae, and nerves, all of which can produce similar symptoms. Sometimes the pain originates within the hip joint itself from arthritis or cartilage irritation. In other cases, tight muscles such as the piriformis can irritate the sciatic nerve, creating pain that travels through the buttock and down the leg.
Hip pain may also be referred from the lower back or sacroiliac (SI) joint, making an accurate diagnosis especially important. Because several different conditions can feel remarkably similar, understanding exactly what's causing your symptoms is the first—and most important—step toward effective treatment.

How Does This Happen, and Who Can Help?
Hip pain develops for many different reasons. Repetitive walking or running, prolonged sitting, sports injuries, falls, arthritis, muscle imbalances, or years of wear and tear can all contribute to discomfort and restricted movement.
Piriformis syndrome is one possible cause. The piriformis is a small muscle deep within the buttock that helps rotate and stabilize the hip. When it becomes tight or irritated, it can place pressure on the sciatic nerve, producing symptoms that closely resemble sciatica—including pain, tingling, or aching that travels down the back of the leg.
Other conditions, including hip osteoarthritis, gluteal tendinopathy, bursitis, SI joint dysfunction, or lumbar spine problems, can produce very similar symptoms. That's why guessing often leads to ineffective treatment.
